As part of the booking process, you will be asked to enter some information. Previously this form would have been completed at your appointment. If you would prefer not to enter this information online please book over the phone and request a paper form at your appointment.

There will be an option to create an account. By doing so, your information will be saved for any future bookings (saving you from completing the form again) and you will be able to reschedule your appointment if needed.
