New Medical Cannabis Patients must complete all 3 steps to book appointment:
1. Upload legal ID showing WA address and fill out the questionnaire.
2. Upload Medical Records showing a qualifying condition (please see greenlightrx.net for qualifying conditions). You must have been evaluated and treated by a physician, chiropractor, or primary care provider. We are required to show that you have been treated with standard treatments besides medical cannabis. Submitting medical records is the fastest way to prequalify.

- Alternatively, you can fill out an Authorization to Release Medical Records, which will be emailed to you for an electronic signature. It can take 1-2 weeks for your health provider to send us your medical records.
3. Prepay for the appointment. We can also accept payment via Venmo or Paypal (please send us an email if you need to pre-pay by Venmo/Paypal. Pre-payment is refundable up to 24 hours before the appointment. No-shows and late cancellations with less than 24 hours notice are charged a late cancellation fee of $50.
4. Once we verify that you have a qualifying condition with supporting documentation from your licensed health care provider, then we will Pre-Qualify you for an authorization and approve your appointment. Appointments will be declined and refunded for patients without proper documentation or without qualifying conditions.
5. You must be seen IN PERSON for a physical exam to receive a new authorization. Renewals (patient has previously been seen in person at GreenLight Rx) may qualify for a telemedicine appointment.
