In order to provide a smooth process, we ask that you please carefully read, note, and follow the following instructions:
Please arrive 15-20 minutes before your appointment time to completely fill out the intake form. It is imperative that you take the time and complete your medical / surgical history, including any medications you are currently taking (as well as herbal or over the counter medications).
Please bring a valid, government issued, picture ID to the appointment. The actual ID must be physically presented at the time of your visit. Please note that we will not accept any ID which we can not scan or photocopy. We will not be able to provide any services without an appropriate identification.
Please continue taking your daily medications, if any, as prescribed. This is especially important for blood pressure medications.
Please note our clinic cancellation policy: a GREATER than 24-hour cancellation time notice is required. Otherwise, we will apply our standard cancellation policy which is to charge a cancellation fee for an amount of $50. You will, however, be able to reschedule your missed or cancelled appointment.
Please note that children or minors can not be left unattended in the clinic.
Please note that you may receive an email as well as text / voice message(s) regarding your appointment.
Please anticipate to spend about one hour in the clinic for your appointment.
