* Receive Social Security? Award letter/bank statement

* 1st time applicants, a hard copy of your SS card or DHS printout verifying SSNs
* Most utility bill(s) (gas/electric)
* DHS current statement ONLY if no household income, or TANF or AABD benefits
* Paying off old bill - lease agreement/ownership proof
* DISCONNECTION NOTICE - CALL don't book
