For Immunization Waivers:

Please complete the virtual waiver education on the waiver section of the LCHD website. After completing the virtual education, you will be prompted to schedule an in-person signature appointment.

All other appointment types:

Select one of the appointment types/times below and answer all questions completely. After answering all the questions, you will click 'Complete Appointment' to schedule your appointment. You will then reach a confirmation page with a summary of the date, time, and location of the scheduled appointment.
