New Patient For patients/clients who have never worked with Dr. Autumn or for returning patients/clients and it has been more than 12 months since a session.
Returning Patient visits are for patients or clients who have worked with Dr. Autumn within the most recent 12 months.
NOTE: If you don't need the full 75 minutes, that is fine. We can utilize whatever time feels appropriate with the time allowed.
If you are unsure, please book as a New Patient.
Late Change Policy
- 48 hours notice is required for any changes to scheduled appointment to avoid a service fee
- $50 may be assessed for any changes less than 48 hours prior to the scheduled appointment.
- The Full Appointment Fee (Up to $188) may be charged for same day changes or missed appointments
- By making an appointment, you automatically agree to these terms.
- Dr. Autumn and Anahata Medicine reserve the right to waive any late change service fee as a courtesy.
- For any changes or concerns in the 48 hour window prior to a scheduled appointment, please call or text Dr. Autumn at 503-709-6226.
- Thank you for your patience and cooperation with this policy. We hope to never assess a late change service fee.
~ The appointment fee is not assessed at the time of booking. We hold a credit/debit card when you schedule to secure the time. You can pay with any method of your choosing at the time of your appointment.
