Our appointment calendar shows two weeks at a time. Appointments are first come first serve. If you do not see any available times please check back in a few days.

Patients are qualified by household income up to 300% of the federal poverty guidelines. Your first appointment will take place by phone.

As you schedule this first phone appointment, please also contact your doctor(s) and ask them to send your prescription(s) to us. This will help speed along the process for us to help you.
