Access Primary Care

To schedule your appointment with Access Primary Care, please choose the appropriate appointment type according to your patient status.

• NEW PATIENT: You will be visiting our primary care clinic for the first time and wish to establish care with one of our providers.

• ESTABLISHED PATIENT: You have been seen at our primary care clinic before and are already under the care of one of our providers.

After choosing your preferred date and time, please provide the required information and confirm your appointment.
