You may fill out the intake form online in step 2, or skip that section and fill it out at the center by arriving 30 minutes prior to your appointment.

You will be sent a confirmation email once your booking is complete.
PAYMENT DETAILS
If you select the Pay Later option you won't be charged until the time of your appointment. If you book an appointment and no show, you may still be charged.
ADDITIONAL APPOINTMENT INFORMATION
Street parking is available with a 2 Hour time limit. Covered parking is available in the garage at Fifth and Orchard/Beaver - rates are $0.75/hour.
