During this appointment we will register you with the Maryland Medical Cannabis Commission. Once your patient ID number has been issued, you will meet with the medical provider to complete the evaluation.

You are a CURRENT patient of New Era. You require assistance with renewing you MMCC patient ID number. Required documents: state ID with current address listed on ID. ID must not be expired. If current address is not listed on ID, please bring TWO ACCEPTABLE proofs of address that show current address. Acceptable proof of address include: MVA registration care or title, MVA change of address care, Utility, telephone or cable/satellite TV bill, Back account statement, property tax bill, Mortgage statement

During this appointment, you will meet with the provider to discuss your medical concerns. You will receive a written certification. We will stay connected to offer guidance on submitting a complete application to the state of Virginia(https://www.license.dhp.virginia.gov/apply/) to help ensure a complete application is submitted to the state. The 50$ state fee is included in the visit fee.

Once the visit with your provider is complete the patient will independently submit the state application by logging into https://www.license.dhp.virginia.gov/apply/Login.aspx to register as a patient .

You have completed and initial visit with New Era. One year has passed and you would like to renew. Please note, if you have never been seen at New Era before, you must complete an initial visit before a renewal visit can be scheduled.
