Please choose the appropriate appointment type for your visit. If you have any questions prior to booking, call our office and a staff member will assist you.
New Patients will be
required to complete New Patient Forms before their scheduled appointment time (forms online do not reflect all
required forms). If you are unable to complete them prior to your scheduled
appointment time, please arrive 30 minutes early to complete the forms
in office. New Patient forms must be completed by your scheduled
appointment time. We want you to have the best experience possible,
therefore, your cooperation with the New Patient Forms and early arrival
is appreciated. In the event you are running late or do not have your
forms completed by your scheduled appointment time, your appointment may
be cancelled or rescheduled (subject to cancellation fee); however, we
will always do our best to accommodate you.
New Auto Accident or Personal Injury:If you are seeking care for injuries from an auto accident or other personal injury, please call to schedule an initial consultation before booking. Auto/Personal Injuries scheduled before having an initial consultation will not be accepted.
Established Patients, please arrive 10 minutes early for check-in.
In the event you need to cancel or reschedule an appointment, please notify us at least 24 hours in advance. Any appointments cancelled within 24 hours of your scheduled appointment time, including no-shows, are subject to a $43 non-refundable cancellation fee. By scheduling your appointment, you are acknowledging and agreeing to the terms of this policy.
For the time being, masks are still required at all times while in the office for both vaccinated and non-vaccinated patients. If you are experiencing symptoms of an illness within 24 hours of your appointment or have been in contact with any person who is positive for COVID-19, we ask that you call us to reschedule.
